Raw Dog Food Diet: Benefits and Risks. Raw dog food diets are controversial. But the popularity of the diets - - which emphasize raw meat, bones, fruits, and vegetables - - is rising. Racing greyhounds and sled dogs have long eaten raw food diets. Extending those feeding practices to the family pet is a more recent idea, proposed in 1. Australian veterinarian Ian Billinghurst. He called his feeding suggestions the BARF diet, an acronym that stands for Bones and Raw Food, or Biologically Appropriate Raw Food. Billinghurst suggested that adult dogs would thrive on an evolutionary diet based on what canines ate before they became domesticated: Raw, meaty bones and vegetable scraps. Grain- based commercial pet foods, he contended, were harmful to a dog’s health. Many mainstream veterinarians disagree, as does the FDA. The risks of raw diets have been documented in several studies published in veterinary journals. Homemade raw food gives you control over what's in your cat's food. It is very economical. It takes a little time and equipment, but it's worth it! Dieting is the practice of eating food in a regulated and supervised fashion to decrease, maintain, or increase body weight. In other words, it is conscious control. Learn why a species appropriate, raw diet is essential in preventing and treating liver issues and liver disease in dogs. Potential benefits of the raw dog food diet that supporters tout include: Shinier coats. Healthier skin. Cleaner teeth. Higher energy levels. Smaller stools. Potential risks include: Threats to human and dog health from bacteria in raw meat. An unbalanced diet that may damage the health of dogs if given for an extended period. I've not lost a lot of weight but I look different, feel different and am different after 6 months of living mostly raw. Potential for whole bones to choke an animal, break teeth or cause an internal puncture. Since Billinghurst’s book,Give Your Dog a Bone, was published, several other types of raw dog food diets have emerged, including commercially processed raw food diets that are frozen or freeze- dried and combination diets that use blends of grains, vegetables, and vitamins that are mixed with raw meat purchased by the owner at the grocery store. Raw dog food recipes and meal suggestions are readily found online and in books. Interest from pet owners continues to grow, with the widespread recall of melamine- contaminated pet food in 2. Raw dog food diet: What it is. A raw dog food diet typically consists of: Muscle meat, often still on the bone. Bones, either whole or ground. Organ meats such as livers and kidneys. Raw eggs. Vegetables like broccoli, spinach, and celery. Apples or other fruit. Some dairy, such as yogurt. Continued“For most animals, it’s more beneficial than processed foods,” says Doug Knueven, DVM, of the Beaver Animal Clinic in Beaver, Pa. Knueven specializes in holistic medicine and also consults for Nature’s Variety, a Lincoln, Neb.- based manufacturer of frozen raw food diets as well as cooked dry and canned foods. Barbara Benjamin- Creel of Marietta started giving raw food to her three dogs after Scooter, a German Shepherd, was diagnosed with cancer. The diet change came too late to help Scooter, she says, but the other dogs are thriving after two years on raw dog food. The 1. 1- year- old dogs seem more energetic, and one with chronic digestive problems tolerates the raw diet better.“The change in the coat was pretty immediate,” Benjamin- Creel says. To cut costs, she stocks up on ground meat when it’s on sale. For a 3. 0- pound dog, a one- day supply of one variety of a frozen, commercially available raw chicken diet costs about $2. A super- premium, commercial dry dog food costs about $1. Raw Dog Food Diet: What the research shows. Lisa M. Freeman, DVM, Ph. D, headed an evaluation of raw dog food diets published in the Journal of the American Veterinary Association in 2. She cautions pet owners against them, saying that many dog owners are choosing raw diets based on online myths and scare tactics about commercial pet food. For pet owners who want to avoid commercial food, Freeman advises a cooked homemade diet designed by a nutritionist certified by the American College of Veterinary Nutrition. Freeman, a nutrition professor at the Cummings School of Veterinary Medicine at Tufts University, says that many of the benefits attributed to a raw food diet for dogs, such as a shinier coat, instead are the result of the high fat composition of the typical raw diet. High- fat commercial foods that would produce the same effect are available, she notes, without the risk of an unbalanced diet. Supplements can also be used as an alternative to increasing fat in the diet. Continued. The evaluation looked at five raw diets, three homemade and two commercially available. All had nutritional deficiencies or excesses that could cause serious health problems when given long term, according to the report. Joseph Wakshlag, DVM, Ph. D, has seen those problems appear in some dogs as poor coats, bad skin, or weak bones. Too little fat means a bad coat; but too much fat and not enough protein can cause mild anemia, says Wakshlag, an assistant professor of clinical nutrition at Cornell University College of Veterinary Medicine. I know the raw food movement gave me some incredible recipes that I would not have found otherwise & still use. I eat a much healthier vegetarian diet than before the. Wakshlag - - who accepts some research funding from Nestle Purina Pet. Care - - says homemade raw diets also may lack enough calcium and phosphorous, causing bone fractures and dental problems. Depending on the quality of the diet, the calcium or phosphorus may also be difficult to properly digest, even if present in adequate amounts. Studies of raw pet food also have shown bacterial contamination. The FDA issued suggestions in 2. A 2. 00. 6 study of 2. These bacteria can also be shed in the feces, leading to a potential source of human exposure and infection. The study also sampled four canned and dry dog foods. Feeding a raw meat diet is no different than cooking chicken for the family .. Raw- meat diets high in liver also may supply too much vitamin A, which can lead to vitamin A toxicity if fed for an extended period. Even veterinarians like Knueven who support raw dog food diets say that they’re not appropriate for all dogs. Because the diets are typically high in protein, they aren’t appropriate for dogs with late- stage kidney or severe liver failure. He recommends that dogs with pancreatitis or other digestive issues start with a cooked, homemade diet and clear up problems before switching to raw. Dogs with cancer, on chemotherapy, or dogs with other immunosuppressive diseases also should not eat raw food. And puppies aren’t good candidates, either.“The only place I’ve seen a problem with this diet is puppies,” Knueven says. Give Your Dog a Bone, self- published, 1. Freeman, L. Metabolism and weight loss: How you burn calories. Metabolism and weight loss: How you burn calories. Find out how metabolism affects weight, the truth behind slow metabolism and how to burn more calories. By Mayo Clinic Staff. You've probably heard people blame their weight on a slow metabolism, but what does that mean? Is metabolism really the culprit? And if so, is it possible to rev up your metabolism to burn more calories? It's true that metabolism is linked to weight. But contrary to common belief, a slow metabolism is rarely the cause of excess weight gain. Although your metabolism influences your body's basic energy needs, it's your food and beverage intake and your physical activity that ultimately determine how much you weigh. Metabolism: Converting food into energy. Metabolism is the process by which your body converts what you eat and drink into energy. During this complex biochemical process, calories in food and beverages are combined with oxygen to release the energy your body needs to function. Even when you're at rest, your body needs energy for all its . Several factors determine your individual basal metabolic rate, including: Your body size and composition. The bodies of people who are larger or have more muscle burn more calories, even at rest. Your sex. Men usually have less body fat and more muscle than do women of the same age and weight, burning more calories. Your age. As you get older, the amount of muscle tends to decrease and fat accounts for more of your weight, slowing down calorie burning. Energy needs for your body's basic functions stay fairly consistent and aren't easily changed. Your basal metabolic rate accounts for about 7. In addition to your basal metabolic rate, two other factors determine how many calories your body burns each day: Food processing (thermogenesis). Digesting, absorbing, transporting and storing the food you consume also takes calories. This accounts for 1. For the most part, your body's energy requirement to process food stays relatively steady and isn't easily changed. Physical activity. Physical activity and exercise — such as playing tennis, walking to the store, chasing after the dog and any other movement — account for the rest of the calories your body burns up each day. Physical activity is by far the most variable of the factors that determine how many calories you burn each day. Metabolism and weight. It may be tempting to blame your metabolism for weight gain. But because metabolism is a natural process, your body has many mechanisms that regulate it to meet your individual needs. Only in rare cases do you get excessive weight gain from a medical problem that slows metabolism, such as Cushing's syndrome or having an underactive thyroid gland (hypothyroidism). Unfortunately, weight gain is complicated. It is likely a combination of genetic makeup, hormonal controls, diet composition, and the impact of environment on your lifestyle, including sleep, physical activity and stress. All of these factors result in an imbalance in the energy equation. You gain weight when you eat more calories than you burn — or burn fewer calories than you eat. While it is true that some people seem to be able to lose weight more quickly and more easily than others, everyone will lose weight when they burn up more calories than they eat. Therefore, to lose weight, you need to create an energy deficit by eating fewer calories or increasing the number of calories you burn through physical activity or both. Sept. Because honey is so rich in sugary carbohydrates, diabetes patients should only eat it occasionally and in small amounts to keep blood sugar levels on track. In a study published in the . Those who had cinnamon experienced lower blood sugar levels after eating than those who did not. The researchers found this to be the result of slower stomach emptying in those who ate cinnamon. The Most Powerful Mercury & Heavy Metal Detox Guide. I could divide my life neatly into two parts: before detox, and after. After, I came alive. Detoxification is, by far, the most important and powerful thing anybody can do to improve their health and their life. Countless articles and books have been written on “detox” and cleansing. Very few of them can be helpful. Most of them however, are pure garbage, and are pure marketing and hype. Detoxification is a long- term, very demanding and challenging process that requires a lot of patience, time, and hard work. You cannot, truly and thoroughly, detoxify your body, with green juices, a supplement, or a 1. I completely turned my life around as a result of detoxifying my body, and helped many people transform their lives by teaching them how to have an optimal diet and detox program. I am not exaggerating when I say that all of my problems in life were directly or indirectly caused by toxicity, from heavy metals and other toxins and opportunistic organisms like candida and parasites. Here, I will share with you what worked best for me and for the people I coached, and show you how to properly detoxify your body from heavy metals and other toxins. According to many readers, this is, by far, the best second- best health article ever written. So, clear- up your calendar, turn off your phone, go to the bathroom if you need to, then go somewhere quiet, and get ready to read one of the most deeply profound and moving articles you’ve ever read. Toxicity – The Obstacle to Reaching Your Fullest Potential. Think of your brain as a bank account. You can constantly keep adding to it, evolving it and growing it, but you can also have a huge negative balance. What you need to do, is not only to keep adding money to your bank account, but what is most important is to get rid of all the unnecessary expenses that are draining your bank account. Imagine that you are making $1. If you can get rid of all or most of those unnecessary expenses, then: 1) you wouldn’t have to work as much or as hard anymore, and 2) even if you keep working, you can actually enjoy and be able to reinvest the money that you are already making. So, eliminating those unnecessary expenses can make a huge positive difference. In this example, your diet (including things like supplements and herbs) and things like exercise, yoga, etc., are the money you are making, and detoxification is the elimination of unnecessary expenses. You can keep improving your diet and do other things to improve your health, and benefit from that, but eventually you’ll hit a plateau. Detoxification is where the real breakthrough can happen and where you can make a big leap and truly transform your health and your brain. It is truly the most profitable long- term investment I have made in myself thus far. Before I did my first real detox, I spent tens of thousands of dollars on my health, and tried every expensive supplement and herb there is, from TA- 6. Chinese herbs like wild ginseng and deer antler, and the results I experienced from only 3 months of detoxification were literally infinitely more profound than the results I experienced from over 5 years of having a very strict diet, taking supplements and herbs, having boring health rituals, doing yoga, meditation, and exercise, all combined. What I realized was this: many of the things we do, such as exercise and yoga, merely offer us a temporary relief from all the toxins we have in our bodies. Once these toxins are out, we really won’t feel the need to do these things. We’ll still benefit from them, but they won’t play nearly as much of a role as they do now. EVERYBODY living in the modern world, is toxic, and can benefit immensely from a detox. Unless you have been living in an island or a jungle for most of your life, you have certainly been exposed to very high levels of toxins and pathogens: heavy metals such as mercury, lead and arsenic, estrogens from plastics, pesticides, chemicals in the environment, parasites, yeast, viruses, mold, microbes, and deadly bacteria. Our immune system and our bodies can handle and detoxify small amounts of these toxins, but in this day and age the load is too high that the body is no longer able to keep up. Nearly 1. 00,0. 00 chemicals or more, many of which are extremely toxic, have been introduced to the environment in the past 1. Our bodies have not yet evolved the detoxification mechanisms to handle such an overwhelming amount of toxicity. The speed at which these toxins are being released is higher than the speed at which our bodies can evolve to handle them. Many people can appear perfectly healthy yet their bodies are host to millions of toxins and pathogens, and their health can infinitely improve by detoxifying and getting some of the garbage out. Learn the different ways on how you can be able to detoxify your body from mercury toxins and achieve a healthier life. Discover the dangers of mercury poisoning brought by dental amalgam fillings in the eye-opening film "Smoking Teeth = Poison Gas". If you think your health is 1. The benefits of a detox on your life are immeasurable and cannot be underestimated. You also need to know that “detox” is another buzz word that is being ignorantly thrown around a lot in reference to very weak and ineffective detoxes such as the master cleanse or a green juice detox. Some of these detoxes can be great for a beginner, but the real life- transforming effects can only be felt from an intensive and structured detox program that targets all of: heavy metals, yeast (candida or fungus), parasites, estrogens, and viruses, which are the underlying root cause behind nearly all health problems. My Own Experience with Detoxification. These are some of the benefits I experienced from detoxification, mainly from heavy metal detoxification. A very hard to describe feeling of unusual, unexplainable wellness, bliss, peacefulness, contentment, joy and unusual calmness. Years ago, before I started learning about health and diet and detoxification, I used to be extremely depressed, negative, sad, nostalgic, needy, and moody, almost all the time. I was so IN my depression and my self- pity, that I couldn’t even consider the possibility that my health had anything to do with how I felt. I thought that something was inherently and fundamentally wrong with me, as if I was broken and flawed by nature of who I am, and that is who I will ever be forever, and the possibility of change was not remotely conceivable to me. I took courses and studied books on personal improvement and positive psychology and did therapy, and tried to change my thoughts and my core beliefs, and “wear” the actions and behaviors of a sane, happy, confident, secure, successful person, which all helped a little bit in the short- term, but deep inside, subconsciously, in the long- term, I always felt like the same piece of shit, and always found myself back at the same hole of darkness. Even as I write this I’m feeling pain just from remembering how painful it was to live like that for years and years over and over again. After about 3 months on a very intensive detoxification program, I started to wake up feeling extremely rested, calm, and just overflowing with positive emotions.
It was like a very heavy burden was on my chest, not allowing me to breathe, for so long, that I just learned to live and adapt with it, and once that burden was lifted, I felt like a blind man who started to see for the first time in my life. My whole outlook on life started to change. I had more motivation to do things, and my appetite to explore and learn new things just exploded. Stupid things did not bother me anymore. I stopped holding grudges against others, or reacting to someone else if they did something I didn’t like or “offended” me, and I had an infinite capacity to just absolutely forget and delete from my memory all the meaningless things that don’t matter, and focus on the 1 or 2 things that really matter. It’s like my memory became much worse at remembering stupid shit and much better at remembering useful things. My whole perception, of everything in life, was changed. To witness yourself actually truly change for the better, from the inside out, is an infinitely exciting experience. On some days I would wake up feeling so good and euphorically calm that I would literally cry for joy. To go from someone who was living in depression and misery and negativity for most of my life to experiencing something like this and starting to become a positive, grateful, optimistic person, was the most powerful, joyful, rewarding, and pleasurable experience I ever had. It is for these feelings of inner peace and calmness, I believe, that we seek religion, love, drugs, and money. We hope that these things will bring us peace and make up for something that we can’t find within ourselves. But what I found is that, without our health and the right biological foundation, true happiness and joy at the deepest levels, are much harder to attain, if not possible at all. Nothing fucks us up spiritually, emotionally, and mentally, more than toxicity. Stronger, sharper intuition. This is also very hard to describe, but what I started to notice after a few months of detox that my intuition became much stronger. I started to get a very strong feeling about things ahead of time, and I would sense whether something is “right” or not. I didn’t want to trust it at first, because, if you’re super excited about something, and all the “data” tell you that it’s a good move, yet your intuition tells you that you shouldn’t do it, it can be a bummer. It is something bizarre, weird, and unscientific, but as I started to trust it more and more, I would find that is never wrong, and whenever I would ignore it, I would pay the price. It is only then that I truly understood what the godfather, Lao Tzu, meant when he said in the Tao Te Ching: Open yourself to the Taothen trust your natural responsesand everything will fall into place. Of course, when I read that years ago, it just sounded like a bunch of gibberish to me. Now, it makes sense more than anything else. Being more “in the flow”, having more creativity, and less writer’s/typer’s block. Mercury Detoxification and Amalgam Fillings Mercury Detoxification: Is It for You? I’ve written this article to introduce the concepts of mercury detoxification. Of course there is much more to this subject than space allows here but it should provide you with enough information to at least consider participating in such a program. You won’t be left in the dark because all the information you will need to understand how chronic mercury poisoning damages the body and how to support your body’s efforts at safely and naturally removing mercury will be found in my book, Mercury Detoxification: The Natural Way to Remove Mercury from Your Body. You can review the Table of Contents and browse excerpts from each chapter. The bookhas a full Glossary, an extensive and detailed Reference section, and a complete Index. It also contains over 5. Tables and Charts. In this article I will review my philosophy and approach to mercury detoxification. I’ve extensively researched this subject and have accumulated over 1. These articles make up the largest organized and abstracted articles on this subject and have an extensive Table of Contents. While I believe my book is the most informative book of its kind on this subject for the layperson, I realize that there are other programs out there. I feel that whether you decide to participate in my program or someone else’s, having a sound understanding of the principles of mercury detoxification will help you to better understand it. My book will do that for you. At least you will have something to compare to and it will serve to make for a more informed choice.(Click Here for information about the basic Mercury Detoxification Supplements I recommend, including the dosage. Although there are more supplements and products involved in my complete mercury detoxification program (see my book) I believe that for any mercury detox to be effective it should include these basic 8 supplements. These can also be taken prior to having your amalgam fillings removed and will help protect the body during- and after- the removal process.) Considering a Mercury Detoxification Program When considering a mercury detoxification program a number of factors should be considered. They are divided into two parts. Subjective Evaluation. Have you been exposed to mercury? Source of exposure. The duration and extent of that exposure. Number and extent of symptoms and diseases related to chronic mercury poisoning. Return to Top. Objective Evaluation. The results of practical mercury testing (optional). While evaluating the factors that are involved in chronic mercury poisoning, the ultimate decision to participate in a mercury detoxification program should be based on the following criteria. Do you want to improve: Your overall health, energy, and be as healthy as you can be? Symptoms that are directly and indirectly related to chronic mercury poisoning? Your immune/detoxification system and increase your resistance to toxins, infections, allergies, and other, more serious diseases? Your quality of life and extend your life- expectancy ? The question is, are you mercury toxic? I believe everyone who still has mercury amalgam (silver) fillings, including those who have had them removed, are mercury toxic to some degree and should seriously consider a mercury detoxification program. Because those who still have these fillings are being constantly exposed to mercury vapor, it's even more important that they begin a mercury detox program as soon as possible. There are two objective tests for mercury that will tell you if you are mercury toxic and provide a general indication of the extent of the toxicity. These are the fecal metals test and the urine mercury challenge (or provoked) test. Both, along with other common tests for mercury, are explained in detail in my book. While I believe testing has a value it isn’t necessary if you fit the subjective criteria for mercury poisoning. But the fecal metals test can also be beneficial. I also feel that an evaluation of your current symptoms and related health issues is a good indicator of the need for a mercury detoxification program. For example, if you have or had amalgam fillings and find you have a number of symptoms, or diseases, related to chronic mercury poisoning, you should seriously consider participating in a program. The following section will serve to explain the importance of symptom diagnosis. When Should I Start a Mercury Detoxification Program? The best answer is to begin a natural mercury detoxification program as soon as possible - especially if you still have mercury amalgam (silver) fillings. Many people have heard or read that they shouldn't start a mercury detoxification program until their amalgams have been removed. This is wrong information. Those who still have amalgam fillings are not only accumulating more and more mercury from them but will continue to become more and more deficient in the essential antioxidants/antitoxins necessary to help the body eliminate mercury. Even when your amalgams have been removed the body will still do its best to remove stored mercury but in the process these essential antitoxins will continue to be depleted. Thus, whether you now have amalgams, or you've had them removed, it is critically important to provide the body with what it needs to increase the production of these antioxidants/antitoxins and improve the health of the immune system as soon as is possible. Symptoms & Diseases Related to Chronic Mercury Poisoning Most people with mercury amalgam fillings aren’t aware of the extraordinary number of symptoms that are directly and indirectly related to chronic mercury poisoning. Mercury just doesn’t do its damage directly, by interfering with the normal metabolic process. It can also devastate the immune system, making you more susceptible to toxins, free radicals, and will lower your resistance to other diseases. Because mercury can be stored in virtually every cell of the body the number of symptoms related to it are wide- ranging. In my opinion, mercury can directly or indirectly cause, contribute to, or make worse every health issue we know of. The list of symptoms and diseases related to chronic mercury poisoning is long, especially when the symptoms related to the indirect of mercury are included. You can find the list by Clicking on Symptoms and Diseases Related to Chronic Mercury Poisoning. Of course, most of these symptoms can also have other causes and contributors. But we do know that every one of them can be made worse by chronic mercury poisoning! This isn’t speculation as there are thousands of studies to support what can happen when exposure to mercury passes the threshold level that will then manifest a related symptom. Also keep in mind that the level of exposure, and the amount of accumulated mercury, can vary from person to person. For example, if 1. For example, the immune/detoxification system of some could be functioning better, allowing them to remove more mercury than those whose system is more compromised. There are also genetic and other factors involved. Of note is that the amount of mercury in the body of those with amalgam fillings has been shown to be proportional to the number of fillings in the mouth. The number and severity of symptoms related to chronic mercury poisoning will be determined by your state of health (especially the status of your immune system), how much mercury you’ve been exposed to over time, and the amount that is stored in your body. Understanding the full effect and depth of chronic mercury poisoning, and how singularly devastating it can be to your health, should also serve to motivate you to consider a mercury detoxification program. You can review an article I wrote on it by Clicking on Chronic Mercury Poisoning and Amalgam Fillings. This is encouraging news and demonstrates how wonderfully the body can respond once the source of the poison has been removed and the body is supported in its efforts at removing mercury. You can review these studies by Clicking on Symptoms & Improvements. You must understand that the key to symptom improvement is directly related to removal of mercury from the body. As long as mercury is still there no related symptom or disease will improve. Removing the most prominent source of mercury, amalgam fillings, is a critical first step and means the body will be better able to remove stored mercury because it no longer as to deal with the mercury entering the body from these fillings. The day you have all of your fillings removed your body will be the most depleted in the antioxidants necessary to increase the rate of removal. Thus, supporting your body’s efforts at removing mercury by initiating a mercury detoxification program will accelerate the removal process and hasten symptom improvement. This rate and extent of improvement is a very individual thing based on a number of factors I discuss in my book. The Confusion about Mercury Detoxification. While there’s a consensus within the mercury free dental community about the need to safely remove mercury amalgam fillings, there’s considerably less agreement about the most effective and safest way to go about removing mercury from the body. I believe a large percentage of dentists and health professionals do not fully understand the effects of chronic mercury poisoning on overall health. I am certain that this lack of awareness has lead to chronic mercury poisoning being extraordinarily under diagnosed. In addition, many mercury free dentists and health professionals who believe in the need and value of mercury detoxification have differing recommendations regarding treating it. Some recommend extremely low dosages of supplements while others recommend dosages that may be too high for many people. This can result in confusion for the layperson, especially when the fundamentals of mercury detoxification haven’t been adequately explained and understood. Hypoglycemia is the most common biochemical finding in the neonatal period. However, despite the common occurrence, there is still controversy on the definition of hypoglycemia in the newborn period. This has led to the development of guidelines designed to identify infants “at- risk” and the implementation of an “operational threshold” for physicians to consider intervention. In healthy term infants, the optimal hormonal and metabolic adaptations during the immediate neonatal period ensure an adequate energy substrate for the vital organs, whereas the abnormal glucose homeostasis observed in preterm and growth- retarded infants is multifactorial in origin. For these high- risk infants, it is important to identify, screen, and prevent significant hypoglycemia. Detailed investigations are warranted in infants with severe and persistent hypoglycemia. Neonatal hypoglycemia is a major cause of brain injury. The speculated mechanisms of cellular injury include excitatory neurotoxins active at N- methyl- D- aspartate receptors, increased mitochondrial free radical generation with initiation of apoptosis and altered cerebral energetic characteristics. This hypoglycemic brain injury predominantly affects parieto- occipital regions causing cognitive, sensory, psychomotor, and behavioral deficits in children. This state of the art review article covers the fetal–neonatal metabolic adaptation, glucose homeostasis in normal and abnormal conditions, management strategies, and late neurological sequelae of hypoglycemia. Perinatal glucose physiology. Fetal blood glucose regulation. During fetal life, glucose is the principal energy substrate. Throughout pregnancy, the fetus receives a supply of substrates essential for growth and for the deposition of fuel stores required after birth through the placental circulation from the mother. Amino acids and lactate in addition to glucose function as energy stores in the fetus. Glucose contributes nearly 6. Maternal hepatic glucose production increases by 1. By facilitated diffusion, glucose is transported across the placenta, but the fetus is capable of endogenous glucose production in the face of placental insufficiency. The placenta transfers to the fetus only 4. The fetus receives high amounts of amino acids through the placenta leading to higher fetal amino acid concentrations than in maternal blood. As pregnancy advances, the amount of glucose transported to the fetus increases, leading to significant deposition of glycogen and fat stores, especially in the third trimester. The fetal blood glucose level normally corresponds to 6. Glycogenic enzymes are present in fetal liver at as early as 8 weeks of gestation and the hepatic glycogen content increases from 3. Amino acids are actively transported to the fetus against a transplacental gradient. The fetal uptake of products of . The fetus has the ability to regulate its blood glucose concentration independently of maternal glucose levels and placental diffusion. In placental insufficiency or maternal starvation, the fetus can activate gluconeogenesis at the expense of growth and storage. On the contrary, suboptimal control of diabetes in the mother will lead to increased secretion of insulin resulting in greater than normal fetal growth. A healthy fetus differs from adults in that there is a blunted insulin response to high glucose concentrations, and that insulin secretion is more sensitive to amino acids than to glucose. The fetus is less sensitive than the neonate to the glycogenolytic actions of glucagon but sensitivity increases with advancing gestational age. In severe and prolonged placental insufficiency, the ensuing failure of glucose homeostasis leads to prolonged fetal hypoglycemia. The fetal liver is enriched with stores to the extent that the liver has three times more glycogen than an adult liver,1. Fetal liver has the enzymes required for gluconeogenesis and glycogenolysis, and in the face of placental insufficiency or maternal starvation these enzymes becomes active. The ketone body production rate is low in fetal life and fetus dependency is more so on amino acid and glucose oxidation for energy rather than on ketones. Fetal endocrine milieu. Insulin dominates the fetal endocrine milieu. Development of the pancreas follows a similar sequence in all mammals, but the stage of gestation at which the specific events occur may vary among species. Insulin remains the main anabolic hormone in fetal life and islet . Intestinal lactase activity is detectable at as early as 8 weeks of gestation in the fetal gut. Lactase activity rises gradually from 8 weeks to 3. With oral feeding in the postnatal period, lactase activity increases in both preterm and term infants. Intestinal lactase activity in the gut is directly related to the gestational age; 2. Metabolic and endocrine adaptation at birth. At birth, the continuous flow of energy substrates from mother to the fetus is curtailed with severance of the umbilical cord. The umbilical cord venous blood glucose concentration is 8. A normal term infant accomplishes this transition via a series of metabolic and hormonal adaptations which allow it an independent existence. This fetal–neonatal adaptation fails to a variable extent in both preterm and small- for- gestational- age (SGA) infants. Endocrine changes occur soon after birth with a decrease in plasma insulin levels and increases of catecholamine and glucagon. The infant switches on the endogenous production of glucose following birth until exogenous nutritional supply is established. After birth, an endocrine stress response leads insulin and glucagon to play a different role in glucose homeostasis, where hepatic glycogenolysis, lipolysis, and . It has been shown that insulin sensitivity is reduced in liver but increased in the periphery in growth- retarded infants. Rate of glucose generation and lipolysis are ineffective in SGA infants but gluconeogenesis from glycerol is efficient. Prior to initiation of breast- feeding, depot fat constituting 1. A high rate of lipolysis occurs in newborn infants soon after birth and is stimulated by thyroid stimulating hormone (TSH) – a dominating lipolytic hormone – immediately after birth. Changes in environmental temperature after birth cause thermal stimulation of the infant’s cutaneous thermoreceptors, leading to activation of the hypothalamic–pituitary–TSH axis, resulting in maximal TSH release, leading to induction of the lipolytic process. Triacylglycerol, rich in depot fat, undergoes hydrolysis and releases nonesterified fatty acids. These fatty acids following hepatic . Lipolysis also generates glycerol, which can enter the gluconeogenic pathway (Figure 1). The metabolism in the newborn depends greatly on the function of vital enzymes: hepatic glycogen phosphorylase for glycogenolysis, phosphoenolpyruvate carboxykinase for gluconeogenesis, and carnitine acyltransferase for ketogenesis. Glucagon, as the major glucoregulatory hormone, induces glycogenolysis, gluconeogenesis, and ketogenesis in liver. Catecholamines, cortisol, and growth hormone do play a key role in glucose homeostasis as counter- regulatory hormones. While on breast feeds, when blood glucose concentrations are low, these infants will have high ketone body levels and the ketone bodies function as alternative fuels, preventing neuronal injury. Formula- fed infants generate low ketone body levels compared to breast- fed infants. Gastrointestinal adaptation at birth. Gastrointestinal adaptation of the newborn is critical for absorption of milk feeds and, therefore, the substrate availability for energy supply to the vital cells, eg neurons. The introduction of milk feeds trigger the secretion of gastrointestinal peptides and hormones, which in turn induce gut growth, mucosal differentiation, and induction of motor activity, facilitating digestion and absorption. Following establishment of a continuous energy source through regular feeds, the infant successfully adapts to an extrauterine life. Glucose physiology in the immediate neonatal period. Following the abrupt cessation of glucose supply to the neonate from the mother at birth, neonatal glucose concentration falls rapidly, reaching a nadir at 1 hour of age and then stabilizing by about 3 hours of age spontaneously or in response to milk feeds in healthy full- term infants. This fall in glucose levels that is noticed after birth appears essential to facilitate the physiological transition for neonatal survival, which includes acceleration of glucose production by gluconeogenesis, stimulation of appetite, adaptation to fast/feed cycles, and promotion of oxidative fat metabolism using lipid from fat stores and ingested milk feeds. Cowett and Farrag reported that the range of plasma glucose concentrations in the neonate at delivery depends on the last maternal meal, the duration of labor, the route of delivery, and the type of parenteral fluid administered to the mother. During this transient hypoglycemic phase, the plasma insulin levels fall and the catecholamine and glucagon levels rise. These vital endocrine changes switch on the essential enzymes for gluconeogenesis, glycogenolysis, lipolysis, and ketogenesis. The low insulin/glucagon molar ratio is critical in neonatal adaptation at birth for glucose homeostasis, resulting from the mobilization of glycogen stores. These changes in pancreatic hormone secretion could be due to a catecholamine surge following the stress of birth. Once the glycogen stores are utilized over the first 1. Definition of hypoglycemia. A numerical definition for neonatal hypoglycemia remains controversial even today. Defining hypoglycemia by a single blood glucose value in the immediate neonatal period is questionable as the blood glucose values are a continuum. The critical level below which the incidence of brain injury increases depends on a number of factors including the availability of alternate energy fuels and the baby’s energy demands.
